Free AI SOAP note generator
Turn rough consultation notes into a clean SOAP note, progress note or referral letter in seconds. Paste your shorthand, pick a format and copy the result into your EHR.
How it works
- 01
Paste your notes
Bullet points, abbreviations or a quick dictation from the visit. Leave out names and ID numbers.
- 02
Pick a format
SOAP note, progress note, referral letter or a plain-language summary for the patient.
- 03
Review and copy
The draft uses only what you wrote. Anything missing is marked “Not documented” instead of made up.
What is a SOAP note?
A SOAP note is the most widely used structure for documenting a clinical encounter. It separates what the patient tells you from what you find, and both from what you conclude and plan to do, so the next clinician can follow your reasoning quickly.
- Subjective
- The patient’s own account: presenting complaint, history of the complaint, relevant past history, medication and allergies.
- Objective
- What you measured and observed: vital signs, examination findings and test results.
- Assessment
- Your clinical impression: the working diagnosis and, where relevant, the differentials you considered.
- Plan
- What happens next: treatment, investigations, advice given, safety-netting and follow-up.
SOAP note example: sore throat
Subjective
42-year-old woman with a sore throat and dry cough for three days. Measured fever of 38.1 °C at home. No shortness of breath. No regular medication, no known allergies.
Objective
Temperature 37.8 °C. Pharynx erythematous without exudate, tonsils not enlarged, no cervical lymphadenopathy. Chest clear on auscultation. Centor score 1.
Assessment
Probable viral upper respiratory tract infection (J06.9).
Plan
Antibiotics not indicated; explained why. Paracetamol or ibuprofen as needed and plenty of fluids. Return if not improving after 7 days, or sooner with difficulty breathing or swallowing.

Tips for better SOAP notes
- Keep the patient’s words in Subjective and your findings in Objective. Mixing them is the most common audit remark.
- Record pertinent negatives you actually checked (“no neck stiffness”) — they show what you excluded.
- Make the Assessment specific enough to code, and state your level of certainty.
- Write safety-netting advice in the Plan: what should make the patient come back, and how soon.
- Document time-critical details such as doses, dates and who was informed.
Frequently asked questions
Is the SOAP note generator really free?
Yes. You can write up to 10 notes a day without an account. A free Docdemic account adds 30 minutes of visit recording and transcription every month.
Is it safe to paste patient information?
Paste clinical details only, never names, ID numbers or contact details. The text is sent to our AI provider to write the note and is not saved by Docdemic.
Will the AI make up findings?
It is instructed to use only what you wrote and to mark missing sections “Not documented”. It is still a draft: read it through before it goes in the record.
Can I paste the note into my EHR?
Yes. Use “Copy note” and paste it into Epic, Cerner, EMIS, SystmOne or any other record system as plain text.
How is this different from Docdemic itself?
This tool rewrites notes you have already typed. Docdemic records the consultation, transcribes it, writes the note in your own templates and suggests ICD-10 codes, so you do not type notes at all.
