Free aesthetic treatment record generator
Turn quick notes from a toxin or filler appointment into a complete treatment record, with product, lot number and units per site kept exactly as you wrote them.
How to use it
- 01
Note consent and screening
Whether consent was signed and photos taken, contraindication screening and previous treatments.
- 02
Paste product and dosing
Product name, lot number, expiry, dilution, and units or volume for each injection site.
- 03
Add outcome and aftercare
How the treatment went, aftercare advice given and when the review is. Lot numbers are never filled in if you left them out.
What goes in an aesthetic treatment record?
Injectable treatments need a precise record: which product went where, in what amount, from which batch, and what the client agreed to. Lot numbers matter for traceability if there is ever a product issue, and units per site make the next treatment easier to plan and adjust.
- Consent and medical screening
- Written consent, photographs, contraindications checked (such as pregnancy, breastfeeding, neuromuscular conditions, medication) and previous treatments and reactions.
- Concerns and assessment
- What the client wants to address and your assessment of the area, for example dynamic or static lines.
- Product details
- Product name, lot number, expiry date and reconstitution or dilution.
- Injection sites and doses
- Each site with units or volume, number of injection points and total dose.
- Immediate outcome
- How the client tolerated the treatment and any immediate reactions.
- Aftercare and review
- Aftercare advice given, what to expect, warning signs and the review appointment.

Tips for aesthetic treatment records
- Record the lot number and expiry for every product used, ideally straight from the vial or syringe label.
- Write units or volume per site and number of points, not just the total; it is what you will need to adjust next time.
- Note the dilution for toxins, so the units recorded can be checked.
- Document consent, photographs and contraindication screening at every treatment, not only the first.
- Record the aftercare and warning signs you gave, and when and how the client can reach you.
- Follow the record-keeping rules of your regulator and any local licensing scheme for cosmetic procedures.
Frequently asked questions
Will it fill in a lot number if I forget?
No. Lot numbers, expiry dates, dilutions and doses are only ever taken from your notes. If one is missing, the draft marks it “Not documented” so you can add it from the product label.
Does it work for dermal fillers?
Yes. Write the product, lot number, volume in ml per area, technique (needle or cannula) and depth, and the draft keeps them as written.
Does it work for skin treatments such as peels or lasers?
It is designed for injectables, but the structure also works for other treatments if you include the product or device, settings and treated areas.
Is it free?
Yes. You can write up to 10 notes a day without an account.
Is it safe to paste client information?
Paste clinical details only, never names, ID numbers or contact details. The text is sent to our AI provider to write the note and is not saved by Docdemic.
