For physiotherapy clinics
AI documentation for physiotherapy clinics
Your hands are busy during treatment, so the note waits until the end of the day. Docdemic records the session and drafts the assessment or follow-up note, plus the exercise sheet for the patient.
Where the documentation time goes
Hands on the patient, not the keyboard
Range of motion, special tests and palpation findings happen mid-treatment. Remembering every value and side until the session ends is hard.
Initial assessments are long
Subjective history, aggravating and easing factors, objective testing, clinical reasoning and goals all need to be documented on the first visit.
The exercise programme is a second document
After the note, the patient still needs a written home programme with sets, reps and how often, in words they understand.
What Docdemic does for you
Assessment and follow-up notes
Docdemic transcribes the session and writes a physio assessment or SOAP follow-up in your template. You check the findings and edit before saving.
Findings you say out loud
Say the measurement as you test it, such as shoulder flexion to 140 degrees on the right, and it lands in the objective section.
Home exercise sheet for the patient
Generate a plain-language exercise sheet from the same session, with the exercises, dosage and instructions you gave.
Between-visit check-ins
Invite the patient to answer questions you choose on their phone between sessions, for example how often they did the exercises. Their answers are summarised for you at the next note.
ICD-10 suggestions
Docdemic suggests ICD-10 codes from the note, checked against the official code list. You confirm them.
Research add-on
If you also do clinical research, an add-on covers literature search, interview transcription and statistics.

Free physiotherapy SOAP note generator →Frequently asked questions
Can it write the home exercise programme?
Yes. It drafts a home exercise sheet from what you prescribed during the session, with dosage and instructions in plain language. You check it before giving it to the patient.
Does it capture range of motion and test results?
It captures what is said during the session. If you say the measurement and the side out loud, it goes into the objective section. Anything you did not say, you add when you review.
What if the session includes a lot of small talk?
The note keeps the clinically relevant content and leaves out the rest. You review the draft before saving.
Are the check-ins a way to monitor patients?
No. They collect what the patient chooses to report between visits, in answer to questions you set, and summarise it for you at the next note.
Can I use my own assessment format?
Yes. The included templates are a starting point, and you can build your own to match how your clinic documents.
