Description
När en patient som under vårdtillfället har fått en läkemedelsgenomgång skrivs ut från sluten vård ska en läkare upprätta en läkemedelsberättelse i patientjournalen. Den ska innehålla uppgifter om vilka ordinationer som har ändrats, vilka andra åtgärder rörande läkemedelsbehandlingen som har vidtagits och orsakerna till de vidtagna åtgärderna. How XV017 is used

Swedish care providers record a KVÅ code for each procedure they carry out and report it, together with the patient’s ICD-10 diagnosis codes, to the National Patient Register. The diagnosis says what the patient has; the KVÅ code says what was done. Medical codes have two letters and three digits and cover investigations, treatments and other care that is not surgery.
