- Description
- Insatser enligt 4 kap. 2 § lagen om samverkan vid utskrivning från sluten hälso- och sjukvård. Planen ska upprättas i enlighet med 16 kap. 4 § hälso- och sjukvårdslagen respektive 2 kap. 7 § socialtjänstlagen. För uppföljning av SIP, se AW010. Utesluter: Upprättande av samordnad vårdplan vid tvångsvård (XU042) och Upprättande av samordnad individuell plan (AU124)
How AU125 is used
Swedish care providers record a KVÅ code for each procedure they carry out and report it, together with the patient’s ICD-10 diagnosis codes, to the National Patient Register. The diagnosis says what the patient has; the KVÅ code says what was done. Medical codes have two letters and three digits and cover investigations, treatments and other care that is not surgery.
